All information on your client consultation form is held with the strictest of confidence. Consultation forms are a requirement for insurance purposes. They provide information on potential contraindications to the treatment being performed.

Treatments may need to be adapted or doctors notes may be required to carry out some treatments.

Consultation form information will never be passed on to a third party. You can ask to see your consultation form and treatment records at any time during working hours.

Any email addresses or contact numbers will be kept strictly for the use of MBHolistics and will only be used in relation to any contact required with regards to treatments and discussions regarding bookings. They will not be passed on for third party use.
